After your initial intake appointment, future, follow up or returning appointments can be scheduled directly with your clinician. Returning SLC@CHC clients can make an appointment with their specialist here.

Did you know CHC offers teletherapy appointments? Teletherapy or virtual appointments may take place via phone or Zoom, and they work for kids, teens, young adults, adult learners, and coaching clients—and is a helpful option for college students who want to continue their care while they are on campus. Ask your therapist whether this option is right for you and your goals.
Contact your specialist directly or contact CHC to cancel, reschedule or change appointments. CHC has a 24-hour cancellation window. If you cancel with more than 24 hours notice you will not be charged.

Whether you come to CHC for individual therapy, a comprehensive evaluation or some combination, our experts help get to the root of your child’s challenges, be they learning, mental health or both. Together with your family, our experts develop a customized and strengths-based approach to care. Because CHC offers a wide variety of specializations in-house, your child’s provider(s) can collaborate with a multidisciplinary team for observation, consultation, care coordination and translation services. CHC is considered an out-of-network provider by all insurance plans. CHC is committed to helping you work with your insurance provider and provides a super bill on a monthly basis. The RISE IOP has been successful in establishing a Single Case Agreement with many insurance providers. If you’re eligible for RISE, you will be connected with CHC staff person who is responsible for establishing SCAs.

We provide expert behavioral health services in both English and Spanish to children, teens and families served by Santa Clara Medi-Cal, ensuring cost isn’t a barrier to getting help. CHC is committed to accessible mental health services for all, regardless of language, location or ability to pay. Some grants are small to allow for many families to qualify. You can request financial assistance at any time, whether you’re just beginning with services or if you’ve had a change in your family’s situation. CHC’s Financial Assistance Application is easy to fill out and we encourage you to apply! Complete the simple two-page application and return it to your Care Coordinator. To determine your eligibility for financial assistance, CHC takes several things into consideration including annual household income, number of individuals supported by your family’s income, living expenses, clinical needs, and special situations facing your family. Evaluations typically include interviews, parent interviews, observations, collaboration with other providers, testing, interpretation, report-writing, and follow-up. Multiple experts. Multiple disciplines. One report. Less confusion.
Once your family has completed an evaluation, participated in the follow up and received a comprehensive report from us, you are invited to return to CHC at no additional cost if you have questions to get further guidance. If you or your child are experiencing a behavioral or mental health crisis, please call or text 988 or head to your nearest emergency room to receive urgent crisis intervention services. While CHC provides mental health treatment to individual experiencing suicidality or self-harm, we are not a crisis clinic and are not equipped to provide same-day appointments to new clients.
