CHC’s Clinical Services Coordinators are the first point of contact for all new families interested in coming to CHC. If you decide to move forward with services, your Clinical Services Coordinator will set up your first appointment, match you with the best available specialist for your child and guide you through the process. When you call or email, you will receive a response within 24 hours.

After your initial intake appointment, future, follow up or returning appointments can be scheduled directly with your clinician.

Please don’t hesitate to contact your Clinical Services Coordinator any time to ask questions about CHC services and resources, update your financial or family paperwork or drop a note about how your child is doing. Did you know CHC offers teletherapy appointments? Teletherapy appointments may take place via phone, Zoom or Skype, and they work for kids, teens, young adults, and parent coaching clients—and is a helpful option for college students who want to continue their care while they are away on campus. Ask your therapist whether this option is right for you and your goals.
Contact your specialist directly or contact CHC to cancel, reschedule or change appointments. CHC has a 24-hour cancellation window. If you cancel with more than 24 hours notice you will not be charged.
At CHC, it is very important to us that your family receives needed services.
At CHC, a multidisciplinary team may care for your child or teen. Your Care Team is carefully chosen based on your family’s set of needs. Because of that, there is no one size fits all offering, and fees are adjusted based on the services being provided. Evaluations carry a standard fee, while different therapy and treatment services, consultation services and other options vary.

CHC is considered an out-of-network provider by all insurance plans. CHC is committed to helping you work with your insurance provider and provides a super bill on a monthly basis. Your Client Billing Team is happy to answer your questions and provide you with paperwork needed to request reimbursements.
The RISE IOP has been successful in establishing a Single Case Agreement with many insurance providers. If you’re eligible for RISE, you will be connected with CHC staff person who is responsible for establishing SCAs.
We provide expert behavioral health services in both English and Spanish to children, teens and families served by Santa Clara Medi-Cal, ensuring cost isn’t a barrier to getting help. We bill services directly to Santa Clara Medi-Cal so there is no cost to the family for our services. Learn more about our services covered by Santa Clara Medi-Cal.
CHC is proud to offer financial assistance to families. Some grants are small to allow for many families to qualify. You can get request financial aid at any time, whether you’re just beginning with services or if you’ve had a change in your family’s situation. CHC’s Sliding Scale Application is easy to fill out and we encourage you to apply! Complete the simple two-page sliding scale application and return it to your Clinical Services Coordinator. To determine your eligibility for financial aid, CHC takes several things into consideration including annual household income, number of individuals supported by your family’s income, living expenses, clinical needs, and special situations facing your family. Your information will be kept confidential.
Once your family has completed an evaluation, participated in the follow up and received a comprehensive report from us, you are invited to return to CHC at no additional cost if you have questions to get further guidance. Children, parents, and families who are involved in court proceedings (eg, custody disagreements, high-conflict divorces) present with unique circumstances that require clinicians who are adept at maneuvering both the mental health system and also the legal system. This is a specialty in the mental health field known as forensic psychology. None of the clinicians at CHC are forensic specialists. We encourage those families who have mental health concerns and legal involvement to reach out to forensic psychologists and neuropsychologists.
